六西格玛管理在留取24 h尿蛋白定量标本流程优化中的应用

摘要

目的:通过六西格玛管理方法对24 h尿蛋白定量标本留取流程进行优化。方法:调查
2016年6至12月需要留取24 h尿蛋白定量标本的1 905例患者中留取失败的原因,应用六西格玛管理法,通过定义、测量、分析、改进和控制5个步骤,优化肾内科24 h尿蛋白定量标本留取流程,将开始实施时(2017年1月)的228例患者与实施半年后(2017年6月)的205例患者进行对比,统计分析改善效果。结果:干预前后,排空膀胱正确率分别为99.12%和100.00%,添加防腐剂正确率为96.93%和100.00%,留取时间正确率分别为98.25%和100.00%,记录尿量正确率分别为98.25%和99.51%;总体上,24 h尿蛋白定量标本留取失败率由原来的7.46%降低至0.49%,差异均有统计学意义(P<0.05)。结论:将六西格玛管理方法应用于肾内科,能优化24 h尿蛋白定量标本留取流程,提升护理服务效率和质量。

Application of Six Sigma as optimization strategy to reduce failure rate of the collection of the 24 h urinary protein (M-TP) specimen

Abstract

Objective: To optimize the collection process of the 24 h urinary protein (M-TP) specimen by Six Sigma method. Methods: The reason for the failure of the M-TP specimen between June 2016 and December 2016 was investigated. Six Sigma management steps includes definition, measurement, analysis, improvement and control, which was applied to optimize the collection process of the M-TP specimen from January 2017 to
June 2017. Results: After application of Six Sigma management, the accuracy rate of bladder emptying increased from 99.12% to 100.00%, the qualified rate of anti-septicadding increased from 96.93% to 100.00%, the accuracy rate of preserving at right time increased from 98.25% to 100.00%, and qualified rate of urine output recording rightly increased from 98.25% to 99.51%. Totally, failure rate of the M-TP preserving decreased from 7.46% to 0.49%. The differences were all statistically significant (P<0.05). Conclusion: The application of the Six Sigma management could effectively optimize the collection process of the M-TP specimen, and improve the efficiency and quality of care services.
