Methods: The clinical data, pathological and immunohistochemical studies of one case of balloon cell nevus were organized. And then we learned its pathological diagnosis and differential diagnosis combined with literature. Results: The congenital nevus on the skin of the chest wall was getting bigger in the past year. The performance of its histology looked as compound nevi with a large number of rich ballooning cytoplasm. The Melan A, S100, HMB45 was positive, CD68 was negative in the balloon-like cells by using immunohistochemistry. Conclusion: Balloon cell nevus is a kind of rarely melanocytic nevi.
