双反牵引闭合复位微创治疗SchatzkerⅡ~Ⅲ型胫骨平台骨折的临床效果

摘要

目的:考察双反牵引闭合复位微创治疗SchatzkerⅡ~Ⅲ型胫骨平台骨折的临床效果。方法:回顾性分析2013年7月至2015年6月我院收治的43例SchatzkerⅡ~Ⅲ型胫骨平台骨折的患者资料。根据治疗方式的不同分为A、B两组,A组患者采用双反牵引闭合复位微创治疗,B组患者采用传统切开复位治疗。比较两组患者手术时间、切口大小、术中出血量、组织损害程度、患者术后下床活动时间以及全负重功能训练时间等指标。患者术后随访1年,评估术后3、6、12个月时患者的膝关节功能。结果:A组的手术时间,切口大小、组织损害程度、术中出血量、下床活动时间均优于B组(P<0.05);全负重功能训练的时间两组无统计学差异(P>0.05)。两组患者术后3、6、12个月时的膝关节功能无统计学差异(P>0.05)。结论:双反牵引闭合复位微创治疗SchatzkerⅡ~Ⅲ型胫骨平台骨折,疗效满意,手术时间短、组织损害小、术中出血量少、下床活动时间早等优点。

Clinical effect of two–way traction closed reduction and minimally invasive treatment for Schatzker type II~III tibial plateau fractures

Abstract

Objective: To investigate the clinical effect of two-way traction closed reduction and minimally invasive treatment for Schatzker type II~III tibial plateau fractures. Methods: 43 cases of Schatzker patients with type II~III of tibial plateau fracture from July 2013 to June 2015 in our hospital were retrospectively analyzed . According to different treatment methods, objects were divided into two groups. Patients of group A were treated with two-way traction closed reduction and minimally invasive treatment, patients of group B were treated with traditional open reduction treatment. The time of surgery, the size of the incision, the amount of bleeding, the degree of tissue injury, the time of postoperative bed activity were compared. The knee joint function was evaluated at 3, 6 and 12 months after surgery. Results: The time of surgery, the size of the incision, the amount of bleeding, the degree of tissue injury, the time of postoperative bed activity of A group were better than that of B group (P<0.05). The time of full weight bearing functional training of the two groups had no significant difference (P>0.05). The knee function at 3, 6 and 12 months had no significant difference (P>0.05). Conclusion: Two-way traction closed reduction and minimally invasive treatment of Schatzker II~III tibial plateau fracture has curative effect, short operation time, less tissue damage, less blood loss during operation, and helps get out of bed earlier and so on.
