代谢综合征合并哮喘患者中TNF-α-308位点单核苷酸多态性的临床意义

摘要

目的:研究代谢综合征(metabolic syndrome,MS)合并哮喘(asthma)患者中TNF- 基因表达及启动子区域-308位点单核苷酸多态性的临床意义。方法:选择2011年6月至2014年六月来本院治疗的MS患者112例(MS组),MS合并哮喘患者78例(MA组)进行研究,选择同期60例健康志愿者作为对照组。测定各组生化指标及肺功能。采用酶联免疫吸附法检测血液中TNF- 蛋白水平。利用real-time PCR检测外周血单个核细胞中TNF- mRNA表达水平,选择聚合酶链反应-限制性片段长度多态性分析方法测定TNF- -308位点单核苷酸多态性。结果:GA+AA基因型频率及A等位基因频率在MS并哮喘病人中明显升高 (P<0.05),MA组GA+AA基因组型与GG基因型比较,体质量指数、低密度脂蛋白、空腹血糖、空腹胰岛素、HOMA-IR、TNF- 含量、IgE、C反应蛋白、内皮素-1水平明显升高;在高密度脂蛋白、PEV1占预计值、PEV1/FVC和一氧化氮水平方面明显降低。TNF- -308位点G/A与体质量指数、舒张压、低密度脂蛋白、空腹血糖、空腹胰岛素、IgE、C反应蛋白、内皮素-1呈正相关,与高密度脂蛋白、HOMA-IR、PEV1占预计值、PEV1/FVC和一氧化氮水平呈负相关(P<0.05)。结论:MS合并哮喘患者TNF- 表达水平增加可能是诱导哮喘发病的危险因素之一。

Clinical significant of TNF-α-308 polymorphism in metabolic syndrome patients with asthma

Abstract

Objective: To study the expression of TNF-α and the function of TNF-α-308 polymorphism in metabolic syndrome patients with asthma. Methods: 112 metabolic syndrome patients without asthma (MS group) treated at our hospital from June 2011 to June 2014 were chose, in the same time, we chose 78 cases with metabolic syndrome complicated with asthma (MA group) and 60 cases of healthy volunteers as control group NC. Biochemical index and pulmonary function of three groups were examined. ELISA was used to detect the protein level of TNF-α. The level of TNF-α mRNA expression in mononuclear cells of peripheral blood was examined by real-time PCR. Fragment length polymorphism PCR analysis method was selected for determination of TNF-α-308 gene polymorphism. Results: The frequency of GA+AA genotype and A allele were significantly increased in MS and asthma patients (P<0.05), GA+AA genotypes and the GG genotype in MA group correlated with higher body mass index, low density lipoprotein, fasting blood glucose, fasting insulin, HOMA-IR, TNF-α, IgE, C alpha in C-reactive protein, endothelin-1 and lower in high density lipoprotein, PEV1 percentage of predicted value, PEV1/FVC and nitric oxide levels. TNF-308 sites (G/A) was positive related with body mass index, diastolic blood pressure, low density lipoprotein, fasting blood glucose, fasting insulin, IgE, C reactive protein, endothelial-1, and negatively correlated with high density lipoprotein, HOMA-IR, PEV1% predicted value, PEV1/FVC and nitric oxide levels (P<0.05). Conclusion: The increase of TNF-α expression and TNF-α-308 gene polymorphism in metabolic syndrome patients may be one of the risk factors inducing asthma.
