直肠腺癌组织中骨桥蛋白与基质金属蛋白酶-3的表达及其临床意义

摘要

目的:探讨骨桥蛋白(osteopontin,OPN)与基质金属蛋白酶-3(matrix metalloproteinase-3,MMP-3)在直肠腺癌组织中的表达及临床意义。方法:采用免疫组织化学EnVision法检测OPN与MMP-3在64例直肠腺癌及癌旁组织中的表达及其相关性,对比分析临床病理特征及随访。结果:OPN与MMP-3在直肠腺癌组织的表达均明显高于癌旁组织,且OPN,MMP-3表达与临床分期、分化程度、淋巴结转移相关(P<0.05);与年龄、性别、浸润深度不相关(P>0.05)。Spearman等级相关分析显示:直肠腺癌中OPN与MMP-3的表达呈正相关(r=0.461,P<0.05)。直肠腺癌临床分期、分化程度及淋巴结转移与OPN,MMP-3阳性表达相关(P<0.05)。OPN与MMP-3阳性患者术后3年总体生存率低于阴性患者(P<0.05)。结论:直肠腺癌OPN与MMP-3表达明显增高,且表达呈正相关,与临床分期、分化程度、淋巴结转移及预后密切相关,有可能成为直肠腺癌重要的生物学标志物,对直肠腺癌诊疗及预后具有重要意义。

Expression of osteopontin and matrix metalloproteinase-3 in rectal adenocarcinoma and its clinical significance

Abstract

Objective: To investigate the expression and clinical significance of osteopontin (OPN) and matrix metalloproteinase-3 (MMP-3) in rectal adenocarcinoma. Methods: Immunohistochemical EnVision method was used to detect the expression and correlation of OPN and MMP-3 in 64 cases of rectal adenocarcinoma and adjacent tissues. The Clinical pathological features were compared and follow-up. Results: The expressions of OPN and MMP-3 in rectal adenocarcinoma were significantly higher than those in adjacent tissues, and the expressions of OPN and MMP-3 were correlated with clinical stage, differentiation and lymph node metastasis (P<0.05). There was no correlation with age, gender, and depth of invasion (P>0.05), and there was no significant difference. Spearman rank correlation analysis showed that OPN was positively correlated with MMP-3 expression in rectal adenocarcinoma (r=0.461, P<0.05). The clinical stage, differentiation degree and lymph node metastasis of rectal adenocarcinoma were correlated with the positive expression of OPN and MMP-3 (P<0.05), and there were significant differences. The overall survival rate of OPN and MMP-3 positive patients was lower than that of negative patients (P<0.05). There was a significant difference. Conclusion: The expressions of OPN and MMP-3 in rectal adenocarcinoma are significantly increased, and the expression is positively correlated. It is closely related to clinical stage, differentiation, lymph node metastasis and prognosis, and may be an important biomarker for rectal adenocarcinoma. OPN and MMP-3 are important for the diagnosis and treatment of rectal adenocarcinoma.
