鼻腔鼻窦黏膜黑色素瘤中CD117,MITF及NAT10的表达及其与临床病理特征、预后的相关性

摘要

目的:探讨鼻腔鼻窦黏膜黑色素瘤(sinuses nasal mucosal melanoma,SNMM)中CD117,MITF及NAT10的表达及其与临床病理特征、预后的相关性。方法:选取2000年1月至2013年12月于中南大学湘雅医院治疗的100例SNMM患者的石蜡标本。采用免疫组织化学法检测SNMM组织中CD117,MITF及NAT10的表达情况,分析其表达水平与临床病理特征及预后的关系。结果:SNMM中CD117,MITF及NAT10的表达与患者年龄、性别、发生部位、临床分期、治疗方式、脑转移均无相关性(P>0.05);MITF及NAT10的表达与肿瘤淋巴结转移有关,MITF及NAT10高表达时肿瘤更易转移(P<0.05),而CD117的表达与淋巴结转移无明显相关性(P>0.05)。CD117的表达与3,5年生存率相关(P<0.05),CD117低表达时患者预后较差;CD117低表达、患者高龄、脑转移是SNMM预后的独立危险因素(P<0.05);MITF与NAT10的表达水平与预后均无相关性(P>0.05);CD117,MITF及NAT10的表达与无病生存期均无明显相关性(P>0.05)。结论:CD117低表达时患者生存率较低,MITF和NAT10高表达时SNMM容易转移;CD117的表达可以作为评估SNMM预后的重要指标,MITF和NAT10的表达可以作为预测肿瘤转移的参考指标。

Expressions of CD117, MITF and NAT10 in sinuses nasal mucosal melanoma and their correlation with clinicopathological features and prognosis

Abstract

Objective: To investigate the expression of CD117, MITF and NAT10 in sinuses nasal mucosal melanoma (SNMM) and its correlation with clinicopathological features and prognosis. Methods: Paraffin specimens from 100 SNMM patients treated in our hospital which was selected from January 2000 to December 2013. Immunohistochemistry was used to detect the expression of CD117, MITF and NAT10 in SNMM tissues, and analyze the relationship between the expression level and clinicopathological characteristics and prognosis. Results: The expression of CD117, MITF and NAT10 in SNMM was not correlated with the patient’s age, gender, location, clinical stage, treatment method and brain metastasis (P>0.05). The expression of MITF and NAT10 was related to lymph node metastasis, and the tumor was more likely to metastasis when MITF and NAT10 were highly expressed (P<0.05), while the expression of CD117 was not significantly correlated with lymph node metastasis (P>0.05). The expression of CD117 was correlated with 3-year and 5-year survival rates (P<0.05), and the prognosis of patients with low expression of CD117 was poor. Low CD117 expression, advanced age and brain metastasis were independent risk factors for SNMM prognosis (P<0.05). There was no correlation between the expression level of MITF and NAT10 and the prognosis (P>0.05). There was no significant correlation between the expression of CD117, MITF and NAT10 and disease-free survival (P>0.05). Conclusion: The survival rate of patients with low CD117 expression was low, and SNMM metastasis was easy when MITF expression was high and NAT10 expression was high. The expression of CD117 could be used as an important indicator to evaluate the prognosis of SNMM, and the expression of MITF and NAT10 could be used as a reference indicator to predict tumor metastasis.
