DNA定量分析在支气管镜刷检及肺泡灌洗液中的应用价值

摘要

目的:对比支气管镜刷检、肺泡灌洗液的液基细胞学与DNA定量分析结果,探讨细胞DNA定量分析在支气管镜刷检及肺泡灌洗液中的应用价值。方法:选取支气管镜刷检+肺泡灌洗液,并进行细胞DNA定量分析与液基细胞学诊断的368例患者,将两者结果与病理活检结果进行对比。 结果:支气管镜刷检细胞DNA定量分析灵敏度、特异度分别为90.8%和79.8%,肺泡灌洗液DNA定量分析的灵敏度、特异度分别为85.8%和78.9%。液基细胞学在支气管镜刷检诊断的灵敏度、特异度分别为68.1%和63.4%,在肺泡灌洗液中的灵敏度、特异度分别为65.9%和60.8%。支气管镜刷检及肺泡灌洗液的细胞DNA定量检测的灵敏度、特异度均高于液基细胞学,二者比较差异有统计学意义(P<0.01)。支气管镜刷检、肺泡灌洗液的细胞DNA定量分析的阳性预测值分别为83.5%和82.4%。结论:细胞DNA定量分析在支气管镜刷检和肺泡灌洗液中有很好的应用价值,同时联合液基细胞学检查能提高恶性肿瘤的检出率,降低漏诊和误诊的风险。

Application value of DNA quantitative analysis in bronchoscopy brush examination inspection and alveolar lavage fluid

Abstract

Objective: To compare the results of quantitative analysis of cytology and DNA of bronchoscope brush and alveolar lavage, and to explore the application value of quantitative analysis of cellular DNA in bronchoscope brush and alveolar lavage. Methods: A total of 368 cases of bronchoscopy and alveolar lavage were selected, and the results were compared with pathological biopsy. Results: The sensitivity and specificity of the quantitative analysis of cellular DNA were 90.8% and 79.8% in bronchoscopy brush fluid, and 85.8% and 78.9% in alveolar lavage fluid, respectively. The sensitivity and specificity of liquid-based cytology in bronchoscopy examination were 68.1% and 63.4% in bronchoscopy brush fluid, and 65.9% and 60.8% in alveolar lavage fluid, respectively, in alveolar lavage. The sensitivity and specificity of the quantitative detection of cellular DNA by bronchoscopy and alveolar lavage were higher than that of liquid-based cytology (P<0.01). The positive predictive values of the quantitative analysis of cellular DNA in bronchoscopy and alveolar lavage were 83.5% and 82.4%, respectively. Conclusion: The quantitative analysis of cell DNA has a good application value in bronchoscope brush and alveolar lavage fluid, and the combination of liquid-based cytology can improve the detection rate of malignant tumors and reduce the risk of missed diagnosis and misdiagnosis.
