摘要

目的:探讨循环胆汁酸(bile acids,BAs)与非酒精性脂肪肝患者骨骼肌容积之间的关系。方法:选 择2012年1至2016年12月在中南大学湘雅二医院就诊的非酒精性脂肪肝病155例患者。检测其血清 BAs及代谢参数,并分析接受营养咨询12个月后患者的血清BAs及代谢参数变化。结果:相关性分 析显示12α-羟基甾醇(12α-OH)BAs(包括脱氧胆酸)水平与上下肢骨骼肌容积和总骨骼肌容积呈负相 关。多因素分析显示脱氧胆酸水平与总骨骼肌容积减少相关,而非12α-OH BAs(包括鹅去氧胆酸)与 下肢骨骼肌容积增加相关。鹅去氧胆酸的变化与下肢Δ骨骼肌容积正相关,与Δ腰臀比和Δ总胆固醇 呈负相关。总非12α-OH BAs水平的变化与下肢的Δ骨骼肌容积正相关。结论:循环BAs与骨骼肌容 积有关,包括脱氧胆酸在内的12α-OH BAs与骨骼肌容积水平降低相关,而非12α-OH BAs包括鹅去 氧胆酸与骨骼肌容积水平增加相关。

Relationship between circulatory bile acids and skeletal muscle volume in patients with non-alcoholic fatty liver

Abstract

Objective: To investigate the relationship between circulatory bile acids and skeletal muscle volume in nonalcoholic fatty liver patients. Methods: A total of 155 patients with non-alcoholic fatty liver disease who were diagnosed in The Second Hospital of Central South University from January 2012 to December 2016 were enrolled in this study. Serum bile acids and metabolic parameters were measured, and serum bile acids and metabolic parameters were analyzed after 12 months of nutritional counseling. Results: Correlation analysis showed that 12α-hydroxysterol bile acid (including deoxycholic acid) levels were negatively correlated with upper and lower extremity skeletal muscle volume and total skeletal muscle volume. Multivariate analysis showed that the level of deoxycholic acid was associated with a decrease in total skeletal muscle volume, whereas the level of non-12α-hydroxysterol bile acids (including chenodeoxycholic acid) was associated with increased skeletal muscle volume in the lower extremities. The changes in chenodeoxycholic acid were positively correlated with the skeletal muscle volume of the lower extremities and negatively correlated with the ΔWHR and Δtotal cholesterol. The change in the total non-12α-hydroxysterol bile acid levels was positively correlated with the skeletal muscle volume of the lower extremities. Conclusion: Circulating bile acids are associated with skeletal muscle volume. 12α-OH bile acids, including deoxycholic acid, are associated with decreased skeletal muscle volume, while non-12α-OH bile acids, including chenodeoxycholic acid are associated with increased skeletal muscle volume. The molecular mechanism underlying the association between bile acid levels and skeletal muscle volume remains to be explored.
