抗阻运动联合经颅直流电刺激对脑卒中偏瘫患者肢体运动功能的影响

摘要

目的:探讨抗阻运动联合经颅直流电刺激(transcranial direct current stimulation,tDCS)对脑卒中偏瘫患者肢体运动功能的影响。方法:将2016年1月至2017年4月粤北人民医院康复科收治的98例脑卒中稳定期患者随机分为3组,其中常规组33例采用常规康复管理方案,电刺激组32例采用“常规康复+tDCS”方案,联合组33例采用“常规康复+tDCS+抗阻运动”方案。比较3组患者肢体运动功能、日常生活能力、神经功能缺损及血清神经肽Y(neuropeptide Y,NPY)和IL-10水平的变化情况。结果:干预后,联合组患者上、下肢Fugl-Meyer运动量表(Fugl-Meyer Motor Scale,FMMS)评分和日常生活能力(activity of daily living,ADL)评分均明显高于对照组与电刺激组,联合组患者神经缺损程度评分量表(Chinese Stroke Scale,CSS)评分明显低于另外2组,联合组NPY和IL-10水平均明显低于另外2组。结论:抗阻运动联合tDCS可有效改善脑卒中偏瘫患者的肢体运动功能,改善其神经缺损症状,且均为无创性操作,值得进一步在临床推广。

Effect of anti-resistance exercise combined with transcranial direct current stimulation on limb movement function of stroke patients with hemiplegia

Abstract

Objective: To investigate the effect of anti-resistance exercise combined with transcranial direct current stimulation on limb movement function of stroke patients with hemiplegia. Methods: A total of 98 cases were randomly divided into three groups in stroke patients in the stable period from January 2016 to April 2017 in our hospital rehabilitation, the conventional group of 33 patients with conventional rehabilitation management scheme, the electrical stimulation group 32 cases with “conventional rehabilitation + transcranial direct current stimulation” scheme, combined group of 33 cases with routine rehabilitation combined with “transcranial direct current stimulation + resistance exercise program”. The changes of limb motor function, activities of daily living, neurological impairment, serum neuropeptide Y (NPY) and IL-10 in three groups were compared. Results: After intervention, the Fugl-Meyer Motor Scale (FMMS) scores and activity of daily living (ADL) scores of the combined group were significantly higher than those of the control group and the electric stimulation group. The Chinese Stroke Scale (CSS) scores of the combined group were significantly lower than those of the other two groups, while the NPY and IL-10 levels of the combined group were significantly lower than those of the other two groups. Conclusion: Resistance exercise combined with transcranial direct current stimulation can effectively improve limb movement function and improve neurological deficit symptoms in patients with hemiplegia after stroke, and all of them are noninvasive operation, which is worthy of further promotion in clinical practice.
