定量检测膀胱尿路上皮癌患者手术前后外周血中survivin mRNA的临床意义

摘要

目的:研究膀胱移行细胞癌患者手术前后全血中survivin mRNA表达变化及其临床意义。方法:采用qRT-PCR检测47名非肌层浸润膀胱癌患者与25名正常人群手术前后外周血中survivin mRNA表达量并构建受试者工作特征(receiver operating characteristic,ROC)曲线。结果:术前膀胱癌患者survivin mRNA表达水平是正常人群的4.06倍(P<0.05),经ROC曲线分析,区别膀胱癌患者与正常人群的survivin表达临界值为1.4×10−2,患者组术后各时间点survivin表达水平明显低于术前水平(P<0.05),survivin mRNA水平高于临界值的术后患者较低于临界值的患者复发率高(P<0.05)。结论:qRT-PCR可简单敏感地检测全血中survivin mRNA水平,survivin可作为用于膀胱癌诊断及复发判断的标志物。

Clinical significance of survivin mRNA detection in the peripheral whole blood of patients with bladder urothelial carcinoma before and after surgery

Abstract

Objective: To investigate the mRNA expression of survivin in whole blood of patients with bladder urothelial carcinoma. Methods: Levels of survivin mRNA expression in the whole blood of 47 patients with non-muscle-invasive bladder cancer before and after operations and 25 normal controls were detected by qRT-PCR and the receiver operating characteristic (ROC) curve for each factor was constructed. Results: The levels of survivin mRNA in bladder cancer patients were 4.06 folds compared to normal controls (P<0.05). The cutoff value for differentiating bladder cancer patients and normal people was 1.4×10−2 for survivin. Postoperative survivin expression in patients was significantly lower than preoperative expression at each time point (P<0.05). Post-operative recurrence rate of post-operative patients with mRNA level above cutoff value were higher than lower ones (P<0.05). Conclusion: qRT-PCR is a convenient and sensitive method to detect survivin mRNA in the whole blood using , and survivin could be useful tumor biomarkers for early detection and recurrence of bladder cancer.
