中青年突发性耳聋患者医患沟通信心的现状及影响因素

摘要

目的:探讨中青年突发性耳聋(sudden hearing loss,SHL)患者医患沟通信心的临床现状,并分析其影响因素。方法:采用连续抽样的方法选择2015年1至12月在广东省中医院耳鼻喉科住院的142名中青年SHL患者为研究对象,并应用一般情况调查表及中文版医患沟通信心量表(patient confidence in communication scale,PCCS)实施调查。结果:中青年SHL患者的PCCS得分为16.41±5.53,条目均分为2.73±0.92,明显低于国内常模的3.76±0.67(P<0.05);多元线性回归分析结果显示,年龄、文化程度、平均月收入及耳聋程度是医患沟通信心的主要影响因素,共可解释52.5%的方差变异量。结论:中青年SHL患者的医患沟通信心相对不足,临床医护人员应加强关注年龄较大、文化程度较低、月收入较低并且耳聋程度较严重的患者,并根据患者自身特点制定针对性的干预策略,以提升医患沟通的有效性和改善临床治疗结局。

Present situation of patient confidence in communicating with doctors for young and middle-aged patients with sudden deafness and its influencing factors

Abstract

Objectives: To investigate the present clinical situation of patient confidence in communicating with doctors for young and middle-aged patients with sudden deafness, and to explore its influencing factors. Methods: A total of 142 young and middle-aged patients with sudden deafness, who were hospitalized in our hospital from January to December in 2015 were investigated by sequential sampling. During the investigation, General Condition Questionnaire and Chinese version of Patient Confidence in Communication Scale (PCCS) were adopted. Results: The PCCS score of young and middle-aged patients with sudden deafness was 16.41±5.53, and the average score of all items was 2.73±0.92, which was significantly lower than that of the national norm (3.76±0.67) (P<0.05). The result of multiple linear regression analysis showed that age, educational level, average monthly income and the degree of deafness were the main factors influencing patient confidence in communicating with doctors, which could explain 52.5% variability of variance components. Conclusion: As for young and middle-aged patients with sudden deafness, their confidence in communicating with doctors was insufficient. Clinicians and nurses should pay more attention to patients with the following characters: older, less educated, lower monthly income and more serious deafness. They should also prepare specific intervention strategies according to patient’s characteristics, to enhance the effectiveness of communication between doctors and patients and improve clinical treatment results.
