小檗碱联合二甲双胍对2型糖尿病血清炎症因子及胰岛功能的影响

摘要

目的:观察小檗碱 联合二甲双胍对2型糖尿病血清炎症因子及胰岛功能的影响。方法:将98例2型糖尿病患者分为2组,对照组49例予二甲双胍口服,治疗组49例予小檗碱联合二甲双胍口服,比较2组治疗前后空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2 hours post-prandial glucose,2hPG)、空腹胰岛素(fasting insulin,FINS)、餐后1 h胰岛素(1 hour postprandial insulin,PINS1h)、餐后2 h胰岛素(2 hours postprandial insulin,PINS2h)、糖化血红蛋白(glycosylated hemoglobin,HbA1c),CRP,IL-6,TNF-α的变化。结果:治疗后,两组患者的FBG,2hPG,FINS和HbA1c均明显降低,且治疗组下降程度优于对照组,差异均有统计学意义(P<0.05)。治疗组治疗前后PINS1h和PINS2h改善明显,且较对照组治疗后显著,差异均有统计学意义(P<0.05)。治疗后,对照组炎症指标CRP和TNF-α下降,差异有统计学意义(P<0.05),IL-6下降不明显,差异无统计学意义(P>0.05);治疗组治疗前后CRP,TNF-α和IL-6下降明显,且改善程度明显优于对照组,差异均具有统计学意义(P<0.05)。结论:小檗碱联用二甲双胍可明显降低2型糖尿病患者的FBG、餐后血糖及FINS,可改善餐后胰岛素分泌;同时减少糖尿病患者体内CRP,IL-6及 TNF-α等炎症因子水平。

Effect of berberine combined with metformin on serum inflammatory factors and islet function in type 2 diabetes mellitus

Abstract

Objective: To observe the effect of berberine combined with metformin on serum inflammatory factors and islet function in type 2 diabetes mellitus. Methods: A total of 98 type 2 diabetic patients were randomly divided into 2 groups. The 49 patients in the control group were given metformin only. The 49 patients in the treatment group were given berberine and metformin. The differences in fasting blood glucose (FBG), 2 hours postprandial blood glucose (2hPG), fasting insulin (FINS), 1 hour postprandial insulin (PINS1h), 2 hours postprandial insulin (PINS2h), glycosylated hemoglobin (HbA1c), CRP, IL-6 and TNF-α were compared between the 2 groups before and after treatment. Results: After treatment, the FBG, 2hPG, FINS and HbA1c levels reduced obviously in the two groups, and the treatment group decreased more, the differences between the two groups was statistically significant (P<0.05). The improvement of PINS1h and PINS2h before and after the treatment was more obvious in the treatment group, the difference was statistically significant (P<0.05). In the control group, the CRP and TNF-α decreased significantly after the treatment (P<0.05). But the decreasing of IL-6 was inconspicuous, there was no statistical difference before and after treatment (P>0.05). However, in the treatment group, CRP, TNF-α and IL-6 all decreased significantly, the treatment group had improved more than that in the control group, the differences were statistically significant (P<0.05). Conclusion: The treatment method of berberine combined with metformin can reduce the level of fasting blood glucose, postprandial blood glucose and fasting insulin, and improve the secretion of postprandial insulin. Meanwhile, it can decrease the level of CRP, IL-6 and TNF-α of type 2 diabetic patients.
